McMillan started his coaching career in 2000, where he was the head coach for the Seattle SuperSonics.
He stayed with the team until 2005, and during his tenure, he made significant contributions that led the team to several victories.
In 2005, McMillan became the head coach of the Portland Trail Blazers, where he continued to showcase his excellent coaching skills. He stayed with the team for seven years until 2012, where he helped the Trail Blazers to secure several playoffs appearances.

- McMillan was an integral part of North Carolina State University’s run to the Elite Eight of the NCAA Championship Tournament in 1985 and 1986, as well as a first-place tie in the Atlantic Coast Conference regular season in 1985.
- McMillan was initially selected by the Sonics as a player out of North Carolina State with the 30th overall pick in the second round of the 1986 NBA Draft.
